This page contains selected HMEPS forms. Below you will find instructions for downloading, printing, and submitting the forms:

The forms are in .pdf format. You will need Adobe Acrobat or Adobe Reader to view and print the forms. Instructions for downloading and printing forms are provided below.

HMEPS must have the original form on file in order for the form to be valid.

Faxed or e-mailed forms will NOT be accepted.

GE01
To be submitted when you are in the process of, or already have moved to, a new address (or one different from the current address HMEPS has on record).
GE12
To be submitted to indicate your marital status.
GE10
To be submitted by those annuitants who wish to have their benefit payments deposited directly into a personal bank account.
GE08
Summarizes the federal tax rules that might apply to payment(s) you are eligible to receive from HMEPS and contains information you will need before you receive your benefits.
RT-2
Provides information for HMEPS members regarding HMEPS's acceptance of rollovers or trustee-to-trustee transfers of funds as payment for HMEPS credited service.
SP04
To be submitted by HMEPS members who wish to change participation from Group B to Group A for future service only.

Request to Purchase Credited Service

SP35

To be submitted by HMEPS members who wish to purchase credited service (e.g., credited service forfeited for a refund, or for which contributions were not made; military service). Note: Conversion of service from Group B to Group A was no longer available after 12/31/05.

DR34
To be submitted by HMEPS members who wish to participate in the Deferred Retirement Option Plan (DROP).

To be submitted by an HMEPS benefit recipient to request a replacement benefit check because the original benefit check was not received in the mail, was lost/destroyed/stolen after being received, or other specified reason.

Instructions for printing the forms:

Once you have the form opened in Adobe Acrobat, click on the "Print" icon (located at the top left of the screen). After clicking on the icon a new window will open indicating your printer settings. Simply click "OK" to print the form.
