Unless otherwise specified below, HMEPS must have the original form on file in order for the form to be valid. (Faxed or e-mailed forms will NOT be accepted.) Please mail forms to:

To be submitted by members who are eligible to retire or by members or former members who have at least five years of credited service and will attain the required age and years of credited service for deferred retirement at a future date.
To be submitted by HMEPS members who wish to purchase credited service (e.g., credited service forfeited for a refund, or for which contributions were not made; military service). Note: Conversion of service from Group B to Group A was no longer available after 12/31/05.
To be submitted when you are in the process of, or already have moved to, a new address (or one different from the current address HMEPS has on record).
To be submitted by those annuitants who wish to have their benefit payments deposited directly into a personal account at a bank or other financial institution.
Summarizes the federal tax rules that might apply to payment(s) you are eligible to receive from HMEPS and contains information you will need before you receive your benefits.
Provides information for HMEPS members regarding HMEPS's acceptance of rollovers or trustee-to-trustee transfers of funds as payment for HMEPS credited service.
To be submitted by an HMEPS benefit recipient to request a replacement benefit check because the original benefit check was not received in the mail, was lost/destroyed/stolen after being received, or other specified reason.
